Where would one purchase the foam?

Any fabric store will have it in a variety of thicknesses. They will cut the length for you but probably not the shape. If you need to adjust it for a perfect fit, an electric meat knife does a REALLY good job.

    Yes, there are stores that sell the kind of foam you’d use for a mattress, they can cut to just about any size within reason. Look in the phone book. If you mean styrofoam insulation, try a building supply place.
